The Role of Level 2 Support Coordination
- Implementation of the NDIS Plan: We assist to implement the various supports and services outlined in your NDIS plan. This involves coordinating and arranging the delivery of services to meet the your needs and goals.
- Provider Liaison: Engage with service providers and connect you with suitable providers to deliver the required supports. They negotiate service agreements, ensuring that the chosen providers align with the participant's preferences and goals.
- Building Capacity and Independence: The primary focus is on building the participant's capacity to exercise choice and control over their NDIS plan. They can support people who find themselves in crisis; however, the focus of the role is crisis prevention and to assist people to build capacity and therefore minimise the risk of crisis reoccurring. Furthermore, they help to navigate the complexities of the NDIS, healthcare, and other systems.
